Droopy eyelids – what causes them and how are they treated?

A “droopy eyelid” is a symptom, not a diagnosis. Heavy eyelids can come from three different problems – and each is treated differently.

The three most common causes

1. Excess eyelid skin (dermatochalasis)

With age, eyelid skin loses elasticity and stretches. The excess forms a fold over the eye and sometimes rests on the lashes, while the lid margin itself sits at a normal height. This is the most common cause of heavy lids after 40, and it is treated with upper eyelid surgery.

2. Ptosis

In ptosis the lid margin itself droops because the muscle that lifts it is weak or stretched. The lid covers part of the iris and sometimes the pupil. Typically the eye looks smaller and sleepy, and you raise your brows to see better. Removing skin alone will not fix it.

3. Low brows

When the brows drop, they push skin onto the upper lid. It looks like excess skin, but the source is higher. A brow lift, often combined with eyelid surgery, may be the answer.

Many people have more than one of these causes – which is why the treatment plan is made after an examination.

A quick self-check

  • Look straight into a mirror with a relaxed forehead. If the lid margin covers the upper part of the pupil, ptosis is possible.
  • Gently lift your brow with a finger. If the hood over the eye largely disappears, low brows play a big role.
  • Lift just the skin fold above the eye. If the lid margin then sits at a good height, the main problem is excess skin.

This is only a rough guide – it does not replace an examination.

Excess eyelid skin is not just about looks

Heavy lids are often more than a cosmetic issue. Excess skin makes make-up difficult, tires the eyes by the end of the day, and habitually raising the brows leads to forehead tension and headaches. The skin in the fold can also become irritated and flaky.

Can droopy eyelids affect vision?

Yes. When skin or the lid margin blocks the upper field of vision, people find it harder to read overhead signs or walk downstairs, and vision improves when they lift the lid with a finger. This can be measured with a visual field test.

Eyelid surgery does not improve sharpness of vision or replace glasses. It can, however, restore a fuller field of vision and reduce eye fatigue if the eyelid was in the way.

When to see a doctor urgently

If an eyelid drooped suddenly – over hours or days – or comes with double vision, headache, unequal pupils or weakness of the face or limbs, seek urgent medical help or go to the emergency department. It may be a sign of a neurological condition. An eyelid that droops much more in the evening than in the morning also needs testing for myasthenia gravis.

Treatment

Creams, exercises or eyelid tapes may improve the look temporarily, but they do not remove excess skin or strengthen the lifting muscle.
